Abstract

We report the case of a 74-year-old patient with known hypertension for 10 years, followed under antihypertensive treatment (Amlodipine 10 mg / day regularly observed), overweight (BMI = 33), with very reduced mobility for osteoarthritis of the knees. She presented with sudden onset solid-liquid dysphagia, accompanied by episodes of coughing upon eating and an infectious syndrome. Faced with this symptomalotogy, she was referred by the cardiologist to the ENT doctor who diagnosed her in addition to dysphagia, laryngitis and angina motivating the realization of an esophageal transit (TO) in search of a foreign body in the tract. upper aerials. This examination objectified an opacification of the entire tracheobronchial tree by iodinated contrast medium (PDCI). In view of these results, an additional cervico-thoracic CT was performed with ingestion of PDCI in search of an esotracheal fistula. We objected to an open, rigid and immobile glottis during swallowing allowing PDCI to pass into the trachea and bronchi, associated with acute bilateral inhalation pneumonia. A complementary cerebral CT without injection of PDC was then performed objectifying an ischemic stroke of the right cerebellar angle, on the territory of the PICA. This lesion was the cause of the X nerve palsy with a defective innervation of the epiglottis responsible for dysphagia, the fossa route and secondarily for inhalation pneumopathy.


RÉSUMÉ 


Nous rapportons le cas d’une patiente de 74 ans, hypertendue connue depuis 10 ans, suivie sous traitement antihypertenseur (Amlodipine 10mg/jour régulièrement respectée), en surpoids (IMC = 33), à mobilité très réduite pour des arthroses aux genoux. Elle a présenté une dysphagie d’installation brutale aux solides et liquides, accompagnée d’épisodes de toux lors de la prise des repas et d’un syndrome infectieux. Devant cette symptomalotogie, elle fut référée par le cardiologue au médecin ORL qui lui a diagnostiqué en plus de la dysphagie, une laryngite et une angine motivant la réalisation d’un Transit œsophagien (TO) à la recherche d’un corps étranger dans les voies aériennes supérieures. Cet examen a objectivé une opacification de tout l’arbre trachéo-bronchique par le produit de contraste iodé (PDCI). Au vu de ces résultats, une TDM cervico-thoracique complémentaire a été réalisée avec ingestion de PDCI à la recherche d’une fistule œsotrachéale. Nous avons objectivé une glotte ouverte, rigide et immobile lors de la déglutition laissant passer le PDCI dans la trachée et les bronches, associée à une pneumopathie aiguë d’inhalation bilatérale. Une TDM cérébrale complémentaire sans injection de PDC a alors été réalisée objectivant un AVC Ischémique de l’angle cérébelleux droit, sur le territoire de la PICA. Cette lésion était à l’origine de la paralysie du nerf X avec un défaut d’innervation de l’épiglotte responsable de la dysphagie, de la fosse route et secondairement de la pneumopathie d’inhalation.
