Abstract

Objective: to study the ultrasound lesions of cirrhosis in chronic carriers of HBV.


Material and method: There were a descriptive cross-sectional study from November 2017 to October 2020 in the health district of Bogodogo. We included all patients with chronic HBV who presented with ultrasound signs of cirrhosis. For each patient, we noted reason for ultrasound, age, taking antiviral treatment or not, size of right liver measured in front of right kidney on a mid-clavicular sagittal section, size of segment IV measured on a recurrent section approximately 1 cm from the anterior hepatic edge, size of left liver measured on a sagittal epigastric section in front of the inferior vena cava, hepatic contours, liver echostructure, presence or not of a hepatic nodule, portal trunk diameter and direction of portal flow as well as its permeability or not, spleen size, presence or not of collateral venous circulation, presence or not of an ascite.


Results: We collected 99 cases of cirrhosis in 3 years out of a total of 3414 ultrasounds performed in chronic HBV carriers, ie an incidence of 2.9%. The mean age of the patients was 39 years with extremes of 09 and 73 years. The sex ratio was 1.4. Only 7% of patients were on treatment. The reason for the ultrasound request was routine follow-up in 89% of cases. The right liver was atrophic in 19% of cases with an average size of 110 mm. Hepatomegaly was found in 11% of cases. Segment IV was atrophic in 60% with an average size of 24 mm. The hepatic contours were irregular in 55% of cases. We noted a granular micronodular echostructure liver in 71% of cases and macronodular in 29% of cases. Portal hypertension was found in 24% of cases with an average portal trunk diameter calculated at 15.5 mm. Thrombosis of the vein was noted in 7% of cases and cavernoma in 4%. Splenomegaly and ascites were objectified in 35% of cases and 22% of cases respectively.


Conclusion: Routine ultrasound monitoring of patients with chronic HBV can detect cirrhosis. Segment IV atrophy, diffuse micro-nodular echostructure and irregular hepatic contours are the most common ultrasound signs. However, this screening remains relatively late because it is often associated at the time of diagnosis with portal hypertension or ascites.

Objectif : Etudier les lésions échographiques de cirrhose chez les porteurs chroniques du VHB.


Matériel et méthode : Il s’agissait d’une étude transversale descriptive menée de novembre 2017 à octobre 2020 au district sanitaire de Bogodogo. Nous avons inclus tous les patients porteurs chroniques du VHB, qui présentaient à l’échographie des signes de cirrhose. Nous avons noté pour chaque patient, le motif de l’échographie, l’âge, la prise d’un traitement antiviral ou non, la taille du foie droit mesurée en avant du rein droit sur une coupe sagittale médio-claviculaire, la taille du segment IV mesurée sur une coupe récurrente à un 1 cm environ du bord hépatique antérieur, la taille du foie gauche mesurée sur une coupe sagittale épigastrique en avant de la veine cave inférieure, les contours hépatiques, l’échostructure du foie,  la présence ou non d’un nodule hépatique, le diamètre du tronc porte et le sens du flux portal ainsi que la perméabilité ou non de celui-ci, la taille de la rate, la présence ou non d’une circulation veineuse collatérale, la présence ou non d’une ascite.


Résultats : Nous avons collecté en 3 ans 99 cas de cirrhose sur un total de 3414 échographies réalisées chez des porteurs chroniques du VHB, soit une prévalence hospitalière de 2,9 % dans le district de Bogodogo. L’âge moyen des patients était de 39 ans. Le sex-ratio était de 1,4 ; 7% des patients étaient sous traitement. Le motif de la demande de l’échographie était un suivi de routine dans 89% des cas. Le foie droit était atrophique dans 19% des cas avec une taille moyenne de 110 mm. Une hépatomégalie a été retrouvée dans 11 % des cas. Le segment IV était atrophique dans 60 % avec une taille moyenne de 24 mm. Les contours hépatiques étaient irréguliers dans 55% des cas. Nous avons noté un foie d’échostructure micronodulaire granuleuse dans 71% des cas et macro nodulaire dans 29% des cas. Une hypertension portale était retrouvée dans 24 % des cas. Une thrombose de la veine porte a été notée dans 7% des cas et un cavernome dans 4%. Une splénomégalie et une ascite étaient objectivées dans respectivement 35% des cas et 22% des cas.  


Conclusion : Le suivi échographique de routine des patients porteurs chroniques du VHB permet de dépister la cirrhose. L’atrophie du segment IV, l’échostructure micronodulaire diffuse et les contours hépatiques irréguliers sont les signes échographiques les plus rencontrés. Cependant ce dépistage reste relativement tardif car il est souvent associé au moment du diagnostic à une hypertension portale ou à une ascite.
