Abstract

Uterine arteriovenous malformations (UAM) are rare but not exceptional.  They are manifested by bleeding of variable importance, sometimes rapidly engaging the vital prognosis of the patient. The aim is to show the interest of imaging in post-partum hemorrhage before management (curettage).


The authors report a clinical case of a 37-year-old woman who presented with bleeding after a voluntary interruption of pregnancy. Ultrasound coupled with Doppler performed in the emergency room suspected a uterine arteriovenous malformation. Magnetic resonance imaging confirmed the diagnosis and allowed embolization with 700µm embozene beads, with a favorable outcome.

Les malformations artério-veineuses utérines (MAVU) sont rares mais non exceptionnelles.  Elles se manifestent par des saignements d’importance variable engageant parfois rapidement le pronostic vital de la patiente. Le but est de montrer l’intérêt de l’imagerie dans les hémorragies du post partum avant tout geste thérapeutique.


Les auteurs rapportent un cas clinique portant sur une jeune femme de 37 ans qui a présentait un saignement survenant après une interruption volontaire de grossesse. L’échographie couplé au Doppler faite aux urgences a suspecté une malformation artério-veineuse utérine. L’imagerie par résonnance magnétique a confirmé le diagnostic et a permis de faire l’embolisation par des billes embozènes 700µm, avec une suite favorable.


Les MAV acquises doivent être recherchées devant l´apparition de métrorragies persistantes dans le post-partum. L’imagerie permet d’éviter le curetage hémostatique (premier recours des obstétriciens) qui serait inutile voire dangereux pour la patiente.
