Abstract





Objective: to present the value of diffusion and perfusion sequences in the early diagnosis of ischemic stroke.


Materials and method: This was a longitudinal prospective multicenter descriptive study over 6 months from July to December 2021. It was carried out in Abidjan in the radiology and medical emergency departments of the University Hospital of Angré. Patients were included on the basis of clinical suspicion of ischemic stroke in the hyper-acute phase. They were explored with a General Electric 1.5 Tesla MRI machine. Axial FLAIR, Gradient Echo, Diffusion slices with ADC mapping, 3D Time of Flight sequences and perfusion sequence were performed. The thickness of the slices was 4 to 5 mm with a field of view of 220 to 240 mm.


Results: It involved 57 patients whose age ranged from 54 to 72 years with a sex ratio of 0.62 (22 men, 35 women) and an average of 67 years ± 4.26. In 51 patients (89.4%), the hyperacute phase infarction was not visible on the FLAIR sequence. On the other hand, on the diffusion sequence, cerebral ischemia was revealed in 53 patients (92.9%) at the hyper acute stage. The infusion made it possible to evaluate the cerebral microvasculature. Fifty-five patients (96.4%) were eligible for intravenous thrombolysis.


Conclusion: The early diagnosis of ischemic stroke affects the prognosis and efficacy of thrombolysis treatment. MRI with infusion and diffusion sequences is the gold standard for exploring hyperacute stroke. The acquisition of high magnetic field MRI machines in several centers in Abidjan has made it possible to establish the early diagnosis of ischemic stroke.


RESUME


Objectif : montrer l’intérêt des séquences de diffusion et de perfusion dans le diagnostic précoce de l’accident vasculaire ischémique cérébral.


Matériels et méthode : Il s’agissait d’une étude multicentrique prospective longitudinale à visée descriptive sur 6 mois allant de Juillet à Décembre 2021. Elle a été réalisée à Abidjan dans les services de radiologie et d’urgences médicales du CHU d’Angré. Les patients ont été inclus sur la base d’une suspicion clinique d’un accident vasculaire cérébral ischémique en phase hyper aigüe. Ils ont été explorés avec un appareil d’IRM General Electric de 1,5 Tesla. Des coupes axiales FLAIR, Echo de gradient, diffusion avec cartographie ADC, des séquences 3D Time of Flight et de perfusion ont été effectuées. L’épaisseur des coupes étaient de 4 à 5 mm avec un champ de vue 220 à 240 mm.


Résultats : Elle a concerné 57 patients dont l’âge variait de 54 à 72 ans avec un sex-ratio de 0,62 (22 hommes, 35 femmes) et une moyenne de 67 ans ± 4,26. Chez 51 patients (89,4 %), l’infarctus en phase hyper aigue n’était pas visible sur la séquence FLAIR. Par contre, sur la séquence de diffusion, l’ischémie cérébrale était révélée chez 53 patients (92,9 %) au stade hyper aigu. La perfusion a permis d’évaluer la micro vascularisation cérébrale. Cinquante-cinq patients (96,4 %) étaient éligibles à la thrombolyse intra veineuse.


Conclusion : Le diagnostic précoce de l’accident vasculaire ischémique cérébral conditionne le pronostic et l’efficacité du traitement par thrombolyse. L’IRM grâce aux séquences de perfusion et de diffusion est l’examen de référence pour l’exploration de l’AVC ischémique à la phase hyper aiguë. L’acquisition des appareils d’IRM de hauts champs magnétiques dans plusieurs centres à Abidjan, a permis d’établir le diagnostic précoce de l’AVC ischémique.
