Abstract

Objective: The aim of this study was to evaluate patient radiation doses during computed tomography (CT) examinations at the Hubert Koutoukou Maga National and University Hospital Center (CNHU-HKM) in Cotonou, in order to establish local diagnostic reference levels (DRLs).

Materials and Methods: This was a descriptive cross-sectional study with both retrospective and prospective data collection, conducted over six months, from July to December 2024, in the CT unit of CNHU-HKM. It included all patients who underwent CT examinations. The department is equipped with a 64-slice FUJIFILM CT scanner with an iterative dose reconstruction software. DRLs were determined using the 75th percentile values of CTDIvol and DLP, and then compared with those reported in the literature.

Results: A total of 718 patients were included, comprising 121 children and 597 adults.
Among children, the mean age was 5.35 ± 4.21 years, with a male predominance (sex ratio 1.52). Only brain CT scans met the required number of cases for establishing pediatric DRLs by age group. The 75th percentile CTDIvol (mGy) values were 24.40, 38.25, 53.86, and 55.28 for the age groups 0–1 year, 1–5 years, 5–10 years, and 10–15 years, respectively. Corresponding DLP (mGy.cm) values were 369.10, 905.97, 1318.60, and 1499.44 respectively for the age groups 0–1 year, 1–5 years, 5–10 years, and 10–15 years.
Among adults (mean age 50.89 ± 16.78 years, sex ratio 1.30), brain CT examinations were the most frequent. The 75th percentile CTDIvol (mGy) and DLP (mGy.cm) values were, respectively: 66.36 and 1709.59 for non-traumatic brain CT; 58.40 and 1658.05 for traumatic brain CT; 38.50 and 1419.98 for lumbar spine CT; 52.00 and 3126.50 for thoraco-abdomino-pelvic CT; 51.50 and 2621.05 for abdomino-pelvic CT; and 53.48 and 768.73 for chest CT.

Conclusion : The radiation doses delivered to patients during CT examinations at CNHU-HKM were higher than those reported in the literature. Optimization measures are therefore necessary, particularly for pediatric examinations.
