Malformation artério-veineuse du cuir chevelu à l’angioscanner cérébral à propos d’un cas au CHU du Point G

Abstract

We report a case of arteriovenous malformation (AVM) of the scalp with the aim of studying the contribution of cerebral CT angiography in the diagnosis. It was a young lady of 20 years old, seen in May 2023 for morphological assessment of an AVM of the skull. On clinical examination, the patient presented at the top of her head with a soft, pulsatile, throbbing, painless swelling with no palpable defect below. This was associated with frontal venous dilation and temporal murmur. Cerebral and cervical CT angiography confirmed the diagnosis of AVM of the left paramedian parietal scalp and identified the arterial afferents (the bilateral carotid artery) and the venous efferents (the bilateral external jugular vein and the median frontal vein). Due to the inadequacy of the technical platform and the patient's lack of financial means, the course of action to be taken was to abstain from treatment. Cranioencephalic and cervical angio-CT is a technique with high sensitivity in the diagnosis of AVM of the scalp and contributes to its management.

Nous rapportons un cas de malformation artérioveineuse (MAV) du cuir chevelu dans

L’objectif d’étudier l’apport de l’angioscanner cérébrale et cervicale dans le diagnostic.

Il s’agissait d’une jeune dame de 20 ans, reçue en mai 2023 pour bilan morphologique d‘une MAV du crâne. A l’examen clinique, la patiente présentait au sommet de sa tête une tuméfaction molle, pulsatile, battante, soufflante, indolore sans défect osseux palpable en dessous. Il s’y associait une dilatation veineuse frontale et un souffle temporal. L’Angio-TDM cérébrale et cervicale a confirmé le diagnostic de MAV du cuir chevelu pariétale paramédiane gauche et a identifié les afférences artérielles (la carotide bilatérale) et les efférences veineuses (la veine jugulaire externe bilatérale et la veine frontale médiane).

Par insuffisance du plateau technique et le manque de moyen financier de la patiente, la conduite à tenir a été l’abstention thérapeutique.

L’Angio-TDM cranio-encéphalique et cervicale est une technique ayant une grande sensibilité dans le diagnostic de la MAV du cuir chevelu et contribue à sa prise en charge.
