Urétéro-hydronéphrose par incarcération de l’uretère au sein d’un muscle grand psoas hypoplasique : A propos d’un cas

We report a case of ureterohydronephrosis due to incarceration of the right ureter in the homolateral psoas major muscle presenting a congenital variant.
Mrs KF, aged 69, with a history of urinary stones, attended the emergency department with acute right lumbar pain. A uroscan showed a right ureterohydronephrosis with a distended right ureter (20 mm in diameter) which made a lateral to IVC curve and became incarcerated between the homolateral leaflets of the psoas major muscle. The superficial layer of the right psoas major muscle was hypoplastic compared with the left psoas major muscle.
No similar case was found in our review of the literature. Hypoplasia of the superficial leaflet of the psoas major muscle is the suspected cause of this variant. It would have allowed the metanephrogenic blastema (embryonic renal stem), during its pelvi-lumbar rise, to slip into the interstice between the superficial and deep leaflets of the psoas major muscle.
Incarceration of the ureter in the psoas muscle is a rare aetiology of ureterohydronephrosis which must be diagnosed.
RÉSUMÉ
Nous rapportons le cas d’une uretéro-hydronéphrose par incarcération de l’uretère droit dans le muscle grand psoas homolatéral présentant une variante congénitale.
Mme KF, 69 ans, qui a des antécédents de lithiases urinaires, s’est présentée aux urgences médicales pour douleur lombaire droite aiguë. L’uroscanner a montré une urétéro-hydronéphrose droite avec un uretère droit dilaté (20 mm de calibre) effectuant une boucle latéro-cave pour s’incarcérer entre les feuillets du muscle grand psoas homolatéral. Le feuillet superficiel du muscle grand psoas droit était hypoplasique par rapport au muscle grand psoas gauche.
Notre revue de la littérature n’a pas permis de trouver un cas similaire. L’hypoplasie du feuillet superficiel du muscle grand psoas est la cause probable de cette variante.
L’uretère incarcéré dans le muscle grand psoas est une étiologie rare d’urétéro-hydronéphrose à diagnostiquer.
