Aspect épidémiologique et facteurs favorisants les incidents et accidents liés à l’injection de produits de contraste iodés lors d’une tomodensitométrie

Objectives: To determine the profiles of patients who have had incidents or accidents after intravenous injection of iodinated contrast media (ICP).Material and methods: Prospective descriptive and analytical study extending over 12 months.Results: Incidents and reactions to ICP were observed after ICP injection in 150 patients out of a total of 5762 patients (2.60%). In the study population, women accounted for 52.7% of patients (79 cases); extravasation of ICP and minor reactions were most often observed in female subjects (63 women / 54 men and 20 women / 16 men); the most affected age group is 60 to 69; minor reactions were observed only at the age of 30 years. When the flow rate was greater than 2ml / s ICP injected, 78.77% (115 patients) had extravasation. ICP was extravagated in all cases where the venous route had been placed for at least 24 hours (89 patients) or on the back of the hand (65 patients). In 68.69% when epi-vein was used for the venous route (68 patients), extravasation was noted.Conclusion: Incidents and accidents related to ICP injection are not negligible in our environments.
Résumé
Objectifs : Déterminer les profils des patients ayant eu des incidents ou accidents après injection intraveineuse de produits de contraste iodé (PCI).
Matériel et méthodes : Etude prospective descriptive et analytique s’étendant sur 12 mois.
Résultats : Les incidents et réactions au PCI avaient été observés après injection de PCI chez 150 patients sur un ensemble de 5762 patients (2,60 %). Dans la population étudiée, les femmes représentaient 52,7% des patients (79 cas) ; l’extravasation du PCI et les réactions mineures avaient été observées le plus souvent chez les sujets de sexe féminin (63 femmes/54 hommes et 20 femmes/16 hommes) ; la tranche d’âge la plus touchée est celle de 60 à 69 ans ; les réactions mineures n’avaient été observées qu’à partir de l’âge de 30 ans. Lorsque le débit était supérieur à 2ml/s de PCI injecté, 78,77% (115 patients) avaient présenté une extravasation. On avait noté une extravasation de PCI dans tous les cas où la voie veineuse avait été placée au moins depuis 24 heures (89 patients) ou au dos de la main (65 patients). Dans 68,69% lorsque l’épi-veine avait été utilisée pour la voie veineuse (68 patients) il avait été noté une extravasation.
Conclusion : Les incidents et accidents liées à l’injection de PCI ne sont pas négligeables dans nos milieux.
