Résumé

Aims: To determine the epidemiological and clinical characteristics and ultrasound diagnoses of bleeding during the first trimester of pregnancy.

Methods: This is a retrospective, descriptive, and analytical study conducted in the Medical Imaging Department of the Mitsinjo Toliara University Hospital. Our study included all pregnant women with metrorrhagia and a gestational age of less than 16 weeks of amenorrhea. Epidemiological, clinical, and ultrasound data were the variables studied.

Results: A total of 122 patients with first-trimester hemorrhage were included in the study out of 1,046 obstetric ultrasounds performed, representing a prevalence of 11.66%. The average age of the patients was 27.89 years, ranging from 14 to 45 years, with a predominance of women aged 20 to 25 years (30.33%). Pregnancies with a gestational age ranging from 7 to 10 weeks accounted for 45.90%. Abortion was the most common cause in 50% of cases, followed by threatened abortion (19.67%), ectopic pregnancy (10.66%), and molar pregnancy (6.56%). Clinical signs (p = 0.000) and gestational age (p = 0.004) were statistically correlated with the ultrasound diagnosis of first-trimester hemorrhage.

Conclusion: Ultrasound is the most widely used and best suited method for the etiological diagnosis of bleeding during the first trimester of pregnancy. In order to improve its management, it is necessary to monitor pregnant women regularly and early on.
