Abstract

Purpose: To list the abnormalities observed on MRI in the exploration of anterior knee pain.


Patients and methods: Prospective descriptive and analytical study, in patients with gonalgia having undergone an MRI during the period from October 1, 2019 to July 31, 2020. The parameters studied were: age, sex, lesions observed and lesion associations. Fisher's Exact test was used to assess the difference in proportion.


Results: The mean age of the patients was 40.4 ± 16.6 years.  There were 49 (58.3%) male patients. Patellar instability and anterior fat contention of the knee were the main lesions observed, affecting 51.2% of patients, respectively. The lesions involved in patellar instability were dominated by patellar chondropathy (41.7%), patella alta (14.3%) and patellar dysplasia (10.7%). The entities found in the conflicts of the anterior fat of the knee included patellofemoral friction syndrome (30.9%) and patellar bursopathy (27.4%). There was a statistically significant association between patellofemoral friction syndrome and patellar instability. Patellar enthesopathy was present in 26.2% of patients.


Conclusion: Patellar instability and conflicts of the anterior fat of the knee are the most common lesions on MRI for anterior knee pain.

But : Répertorier les anomalies observées à l’IRM dans l’exploration d’une gonalgie antérieure.


Patients et méthodes : Etude étude prospective descriptive et analytique de 82 patients (84 genoux), souffrant de gonalgie antérieure et ayant bénéficié d’une IRM durant la période du 1er octobre 2019 au 31 juillet 2020. Les paramètres étudiés étaient : l'âge, le sexe, les lésions observées et les associations lésionnelles. Le test Exact de Fisher a été utilisé pour évaluer la différence en proportion.


Résultats : La moyenne de l’âge des patients était de 40,4±16,6 ans. Il y avait 49 (58,3%) hommes. L’instabilité patellaire et les conflits de la graisse antérieure du genou étaient les principales lésions observées, affectant respectivement 51,2% des patients. Les lésions impliquées dans l’instabilité patellaire étaient dominées par la chondropathie patellaire (41,7%), la patella alta (14,3%) et la dysplasie patellaire (10,7%).  Les entités retrouvées dans les conflits de la graisse antérieure du genou comportaient le syndrome de friction fémoro-patellaire (30,9%) et la bursopathie patellaire (27,4%). Il existait une association statistiquement significative entre le syndrome de friction fémoro-patéllaire et l’instabilité patellaire.  Une enthésopathie patellaire était présente chez 26,2% des patients.


Conclusion : L’instabilité patellaire et les conflits de la graisse antérieure du genou sont les lésions les plus fréquentes à l’IRM pour gonalgies antérieures.
