Abstract

Introduction: Early detection is the key step in the management of breast cancer. The feasibility of an organised screening programme depends on the adherence of the target populations. Our objective was to study the perception and acceptability of breast cancer screening by mammography.


Materials and methods: Prospective, descriptive, and analytical study over a period of 6 months. Women aged 35 years and over, who had never had breast cancer and who were received in the radiology department of the CHU of Treichville (urban population); and at the general hospital of Yakassé-Attobrou (peri-urban population of the Abidjan suburbs) were included. We recorded 518 female respondents.


Results: The average age of the respondents was 41.45 ± 7.73 years with extremes of 35 and 70 years. Mammography compliance in our series was 70.46%. The majority of the respondents had no knowledge of the signs of breast cancer (63.32%) and did not know the risk factors (68%). The factors that made mammography acceptable were: menopause (p=0.02), senior or middle management (p=0.005), urban residence, the existence of health insurance (p=0.011), having had a previous mammogram (p<0.001), the media (p=0.003) and health personnel (p=0.016). Resistance factors were ignorance of the existence of a mammography center (p=0.018), apparent good health and lack of income.


Conclusion: The perception of breast cancer and its risk factors was erroneous and should be corrected by awareness raising. The acceptability of mammography was good but would be improved if mammography screening was subsidised by the state.

Introduction : Le dépistage précoce représente l’étape clé dans la prise en charge du cancer du sein. La faisabilité d’un programme organisé de dépistage passe obligatoirement par l’adhésion des populations cibles. Notre objectif était d’étudier la perception et l’acceptabilité du dépistage du cancer du sein par la mammographie.


Matériels et méthodes : Etude prospective, descriptive et analytique sur une période de 06 mois. Ont été inclus des femmes âgées de 35 ans et plus, n’ayant jamais eu de cancer du sein et reçus dans le service de radiologie du CHU de Treichville (population urbaine) ; et à l’hôpital général de Yakassé-Attobrou (population péri-urbaine de la banlieue abidjanaise). Nous avons enregistré 518 enquêtées.


Résultats : L’âge moyen des enquêtées était de 41,45 ± 7,73 ans avec des extrêmes de 35 et 70 ans. L’adhésion à la mammographie dans notre série était de 70,46%. Les enquêtées méconnaissaient les signes du cancer du sein (63,32%) et n’en connaissaient pas les facteurs de risque (68%). Les facteurs d’acceptabilité de la mammographie étaient : la ménopause (p=0,02), la profession de cadre supérieur ou moyen (p=0,005), le lieu de résidence urbain, l’existence d’une assurance maladie (p=0,011), la réalisation d’une mammographie antérieure (p<0,001), les médias (p=0,003) et le personnel de santé (p=0,016). Les facteurs de résistance étaient l’ignorance de l’existence d’un centre de mammographie (p=0,018), la bonne santé apparente et l’absence de revenus.


Conclusion :La perception du cancer du sein et de ses facteurs de risque par les enquêtées étaient erronées et devrait être corrigées par la sensibilisation. L’acceptabilité de la mammographie était bonne mais elle serait améliorée si le dépistage par la mammographie était subventionné par l’état.
