Abstract

Objective: to evaluate the performance of computed tomography (CT)-guided and ultrasound-guided percutaneous biopsy of abdominal and thoracic lesions.


Method: it was about a cross-sectional, descriptive and analytical study with prospective data collection over a period of 11 months, from december 1, 2020 to October 31, 2021; at the Clinique Autel d’Elie (CAE). The study population consisted of all patients in whom an ultrasound or CT-guided biopsy had been performed.


Results: twenty-five (25) patients were included. The mean age of the patients was 53,16± 12,30 years. The female sex was the most represented (56%). The CT scan was performed before the biopsy in 23 patients (92%) and the ultrasound in 06 patients (17.1%). A primary lesion was suspected on imaging in 76% of cases and a secondary lesion in 24% of cases. Seventy-two (72) % of suspected tumor lesions were malignant and dominated by hepatocellular carcinoma (HCC), lymphoma and pancreatic adenocarcinoma. Eighty (80) % of the biopsied masses were located in the abdominal region and 20% in the thoracic region. Biopsy guidance was performed by CT scan in 19 patients (76%), and ultrasound in 06 patients (24%). The main complication recorded after the biopsy was hemoperitoneum in 06 cases (24%). Histology found malignant lesions in 62.5% of cases, secondary lesions (29.2%) and benign lesions (8.3%). The accuracy of the biopsy was 96%. Histology confirmed the suspected diagnosis in the pre-procedural imaging in 20 patients (83.3%) and invalidated it in 04 patients (16.7%).


Conclusion: this study had shown a clear efficacy of radioguided biopsy in the diagnosis of abdominal or thoracic masses in adults.

Objectif : évaluer l’apport de la biopsie radioguidée dans le diagnostic des masses thoraciques et abdominales.


Cadre et méthode : il s’est agi d’une étude transversale, descriptive et analytique avec collecte prospective des données sur une période de 11 mois, allant du 1er décembre 2020 au 31 octobre 2021 ; à la Clinique Autel d’Elie (CAE). La population d’étude était constituée de tous les patients adultes chez qui avait été réalisée une biopsie écho ou scanoguidée.


Résultats : vingt-cinq (25) patients avaient été inclus. L’âge moyen des patients était de 53,16± 12,30 ans. Le sexe féminin était le plus représenté (56%). Le scanner avait été réalisé avant la biopsie chez 23 patients (92%) et l’échographie chez 06 patients (17,1%). Une lésion primitive avait été suspectée à l’imagerie dans 76% des cas et une lésion secondaire dans 24% des cas. Soixante-douze (72) % des lésions tumorales suspectées étaient malignes et dominées par le carcinome hépatocellulaire (CHC), le lymphome et l’adénocarcinome pancréatique. Quatre-vingt (80) % des masses biopsiées siégeaient à l’étage abdominal et 20% à l’étage thoracique. Le guidage de la biopsie a été fait au scanner chez 19 patients (76%), et à l’échographie chez 06 patients (24%). La principale complication enregistrée après la biopsie était l’hémopéritoine dans 06 cas (24%). L’histologie avait retrouvé des lésions malignes dans 62,5% des cas, les lésions secondaires (29,2%) et bénignes (8,3%). L’efficacité diagnostique (accuracy) de la biopsie était de 96%. L’histologie avait confirmé le diagnostic suspecté à l’image pré-procédurale chez 20 patients (83,3%) et l’avait infirmé chez 04 patients (16,7%).


Conclusion : cette étude avait montré une nette efficacité de la biopsie radioguidée dans le diagnostic de masses thoraciques ou abdominales de l’adulte.
