Abstract

Congenital malformations of the thoracic aorta and its branches are rare and diagnosis is usually incidental. They are often associated with other cardiovascular anomalies. We report two consecutive cases of dextro-transposition of the thoracic aorta.


 


The first patient was an 80-year old male, hypertensive with poor follow-up whose chest X-ray showed a right-sided aortic knob. The second patient was a 47-year old male with no relevant previous medical history who consulted a cardiologist for chest pain. Both underwent chest CT angiography which revealed a right aortic arch (dextro-transposition) associated with an aberrant left subclavian artery arising from a Kommerell diverticulum


 


Anatomical variations of the thoracic aorta are often responsible for compression phenomena on the nearby organs, particularly on the esophagus (dysphagia lusoria). Medical imaging plays a key role in the assessment of anatomical variations of the thoracic aorta, with CT


RESUME


Les malformations congénitales de l’aorte thoracique et de ses branches sont rares et de découverte fortuite. Elles sont souvent associées à d’autres anomalies cardiovasculaires. Nous rapportons deux cas d’aorte thoracique à droite.


 


Il s’agit de deux sujets de sexe masculin, l’un âgé de 80 ans, hypertendu mal suivi dont la radiographie du thorax a montré un bouton aortique à droite et le second, âgé de 47 ans, sans antécédent particulier, a consulté en cardiologie pour l’exploration d’une douleur thoracique. Les deux ont bénéficié d’un angioscanner thoracique qui a révélé une crosse de l’aorte thoracique à droite associée à une artère sous-clavière gauche aberrante issue d’un diverticule de Kommerell.


 


Ces variations anatomiques de l’aorte thoracique sont souvent responsables de phénomène compression sur les organes de voisinage notamment sur l’œsophage (dyspnée lusoria). L’imagerie en coupe notamment de l’angioscanner thoracique permet le diagnostic de ces malformations.
