Abstract

The aim of this case report was to highlight the interest of imaging in the management of juvenile pheochromocytoma at the G point CHU, Bamako.


We report a case of pheochromocytoma in a 16-year-old adolescent with a history of severe hypertension without treatment. The diagnosis was directed by the clinic. The abdominal scanner revealed a large right adrenal mass with a necrotic center measuring 79.7 x 52.2 mm, the spontaneous density of which is measured at 35 HU, the density at the early stage at 60 seconds measured at 75 HU and the late density at 10 minutes after the intravenous injection of contrast product was 52HU. Thus the relative and absolute "Wash out" was evaluated at 6% and at 39%. The diagnosis of pheochromocytoma was made on the basis of the determination of normetanephrine in the urine, of urinary methoxy-tyramine and especially of the determination of urinary vanyl-mandelic acid (VMA) and confirmed by the pathological examination of the surgical specimen. The outcome was favorable after tumor resection with normalization of blood pressure figures. The CT scan 6 months after the tumor is removed (pheochromocytoma) does not indicate tumor recurrence.

Le but de ce rapport de cas était de faire ressortir l'intérêt de l'imagerie dans la prise en charge du phéochromocytome juvénile au CHU point G, Bamako.


Nous rapportons un cas de phéochromocytome chez un adolescent âgé de 16ans avec antécédent d’hypertension artérielle sévère sans traitement. Le diagnostic a été orienté par la clinique. Le scanner abdominal avait mis en évidence une grosse masse surrénalienne droite à centre nécrosé mesurant 79,7 x 52,2 mm dont la densité spontanée est mesurée à 35 UH, la densité au temps précoce à 60 secondes mesurée à 75UH et la densité tardive à 10mn après l’injection intraveineuse de produit de contraste était de 52UH. Ainsi le « Wash out » relatif et absolu a été évalué à 6% et à 39%. Le diagnostic de phéochromocytome a été posé sur la base du dosage de normétanéphrine dans les urines, du méthoxy-tyramine urinaire et surtout du dosage de l’acide vanyl-mandélique (VMA) urinaire et confirmé par l’examen anatomopathologique de la pièce opératoire. L’évolution était favorable après exérèse de la tumeur avec normalisation des chiffres tensionnels. Le contrôle scanographique à 6mois d’exérèse de la tumeur (phéochromocytome) était normal.
