Abstract

Goals: Describe the different types of ear defect on computed tomography.


Materials and Methods:  Retrospective study collected at the pediatric radiopediatrics department of Chu Ibn Rochd in Casablanca over a period of 15 months having consulted for rock malformations. A multislice scanner with fine-sectional reconstruction in the 3 planes without injection of contrast product was used for carrying out the examinations.


Results: The male sex was dominant (10 boys / 5 girls), the average age was 7 years.


The circumstances of discovery were defects of the pinna (7 cases), deafness (6 cases) and facial palsy (2 cases). CT revealed the following malformations: the outer ear (14 cases, or 48.28%), the middle ear (9 cases, or 31.03%) and the inner ear (6 cases, or 20.69%). These were: Vestibular ectasia (n = 3), agenesis of the external auditory canal (EAC) (n = 7), hypoplasia of the EAC (n = 1), hypoplasia of the eardrum (n = 3), hypoplasia ossicles (n = 4), uncudo-malleal weld with horizontal arrangement of the anvil and hammer (n = 1), hypoplasia of the lateral semicircular canal (CSCL) (n = 1), agenesis of the second portion of the facial nerve (F2) (n = 2), thin aspect of the internal auditory canal (IAC) (n = 1), dilation of the aqueduct of the vestibule (n = 2). There was no aberrant F2 path or modiolus dilation.                                                                                                                 


Conclusion: Ear malformations seen on a computed tomography in our context predominate on the outer ear. They are mostly made up of agenesis of the CAE and hypoplasia of the ossicles and the eardrum.

Objectifs : Décrire  les différents types de malformation de l’oreille  en TDM.                                                                                                                                      Patients et méthodes : Etude rétrospective colligée au service de radiopédiatrie du Chu Ibn Rochd de Casablanca sur une période de 15 mois ayant consulté pour  malformations des rochers. Un  scanner multibrarrette avec reconstruction en coupe fine dans les 3 plans sans injection de produit de contraste  a servi pour la réalisation des examens.


Résultats : Le sexe masculin était dominant (10 garçons/5 filles),  l'âge moyen était de 7 ans.  Les circonstances de découverte étaient  les malformations du pavillon de l’oreille (7 cas), la surdité  (6 cas) et  la paralysie faciale (2 cas). La TDM  a mis en évidence des malformations suivantes: l’oreille externe (14cas soit  48,28  %) l’oreille moyenne (9 cas soit 31,03 %) et l’oreille interne (6 cas  soit 20,69 %). Il s’agissait de : Ectasie vestibulaire (n=3), agénésie du conduit auditif externe ( CAE) (n=7), hypoplasie du CAE (n=1), hypoplasie de la caisse du tympan ( n=3), hypoplasie des osselets (n=4), soudure uncudo-malléaire avec disposition horizontale de l’enclume et du marteau (n=1), hypoplasie du canal semi-circulaire latéral (CSCL) (n=1), agénésie de la deuxième portion du nerf facial (F2) (n=2), aspect grêle du conduit auditif interne (CAI) (n=1), dilatation de l’aqueduc du vestibule (n=2 ). Il n’y avait  pas  de  trajet aberrant de F2 ni de  dilatation du modiolus.


Conclusion : Les malformations de l’oreille vues au scanner dans notre contexte, prédominent sur l’oreille externe. Elles sont en majorité constituées d’agénésie du CAE et d’hypoplasie des osselets et de la caisse du tympan.
